Taylor McCallum (Mac) Black, a fourth year medical student at UW, presented at our March forum.

He examined what experiences motivate people in medical education: what works for and against them? Specifically he developed and discussed data concerning how important a course for medical students about service learning is in achieving the School’s mission of meeting the needs of the region and the underserved.

Many thanks to our last speaker, Andrew White, and discussant guests, Sarah Shannon and Peggy Odegard, for a rich and interesting forum discussion on medical error.

Andrew's talk was entitled: How Trainees Perceive and Disclose Medical Error. I will post his slides shortly. Much of the discussion was about interprofessional aspects of team errors, and about team error disclosure.

The 2009 American Association of Medical Colleges annual meeting was held in Boston in early November.. and was attended by many of our UW Med Ed Forum frequent fliers. We reviewed major themes from the meeting and had a meaty discussion, including the following topics:

-- e-learning: national focus on research in instructional design
-- comparative effectiveness research on educational interventions is ripe for funding
-- simulation: outcomes of open exploration vs. proficiency based criteria vs. highly prescriptive approach
-- study of process and outcome within medical education
-- increasing attention to integration of basic sciences and clinical medicine (out of this sprang a discussion of the content and process that basic sciences bring to to medical school... and how to optimize learning of this material; discussed drawing basic sciences through the 4 years, and starting clinical work earlier?)
-- physician wellness / mindfulness / satisfaction (faculty and residents)

Growing a WWAMI community

Dr. Peggy Schlesinger, who has participated in the Med Ed forum via conference call from Montana so many times this year, joined us in Seattle in June and gave an outstanding talk on Growing a WWAMI community in Missoula Montana -- thanks, Peggy!

WWAMI is an acronym for the five states joined in a medical education partnership with the University of Washington --Washington, Wyoming, Alaska, Montana and Idaho.

Peggy is a private practice physician in internal medicine and rheumatology in Missoula, Montana, has taught UW medical students in Missoula since 1992, and is coordinator for WWAMI internal medicine clerkship for third-year medical students.

5/5/2008 - The Evolution of Professionalism in Residency Training

On May 5, 2008, Molly Billings, senior fellow in the Division of Pulmonary and Critical Care Medicine, Department of Medicine, presented plans for her research on the Evolution of Residents' Professionalism during Training.

Molly reviewed the literature on professionalism, burn-out, cynicism, and empathy and laid out plans for her research to assess the linkages between professionalism with burnout and empathy and the interaction of cynicism and empathy. She intends to complete a longitudinal study of UW internal medicine residents in years 1 through 3, assessing over time these associations using a number of instruments, including the Jefferson Scale of Physician Empathy.

During the feedback period, participants provided suggestions for her research, primarily related to methodology, such as the need to carefully pilot the surveys, assessing the number of questions used and the length overall of the survey, etc.

Great turn-out and this time we had larger plates that the food didn't drop off of and a non-Rice Crispies dessert!

The next group will held be Monday, June 2 from noon to 1:15 in South Campus Center Room 348. Faculty from the Department of Medical Education and Biomedical Informatics will provide information on resources and expertise within the department.

4/7/2008 - Cynicism and Satisfaction in Medicine

At our last Med Ed Forum gathering on 4/6/2008, Molly Blackley Jackson, Acting Instructor in the Department of Medicine, presented her research project entitled Culture Change: Cynicism and Satisfaction in Medicine.

We reviewed the history of publications on cynicism in medicine, and talked about related publications (including empathy, burnout, and measures of professional satisfaction). She then discussed her research project, in which she and colleagues conducted 30 extensive interviews (10 each with medical students, residents, and attendings at UW) with open ended questions about professional expectations, personal degree of cynicism and suspected etiologies, satisfaction, and best moments in medicine.

We discussed and gave feedback on the early conceptual model created from this data (see slide 21 in the ppt), and discussed further research in this arena (including collecting more broad quantitative data from these qualitative data, expanding the project to include community medicine physicians, considering how cynicism and satisfaction similarly affects other fields, including the basic sciences, nursing, engineering, law, etc).

The turnout was fantastic, and a lively discussion followed. Many thanks to those who participated -- next time we'll order more food and larger plates!

3/3/08 - Dr. Glen Tamura & Journal Club

On Monday, 3/3/08, Glen Tamura, one of the UW Colleges faculty members, presented on his research project, Evaluation of Student Case Reports, focusing on the reliability of faculty physician assessments of medical student write-ups. His purpose is to evaluate the performance of faculty as teachers, assessing feedback that faculty give to students on their writeups (on what write-up sections do faculty members comment? what types of errors do they focus on? were comments mostly positive or negative? were they specifically constructive or general? is there consistency between faculty members?). He reviewed the literature for us on previous research in this area (there hasn't been much -- but all report wide variabilty among physicians when critiquing/grading write-ups).
He then reviewed his ongoing project, in which he crafted 3 "student" write-ups to include common errors. He is in the process of having each College faculty member (there are 30; including ongoing/experienced and new faculty) comment and give a final evaluation grade for the write-up. His hypothesis is that the ongoing College faculty more reliably assess write-ups when compared to new College faculty. He would like to extend this project to clerkship faculty who teach in the third year.

The group had a great discussion and provided some ideas and feedback to Dr.Tamura on the research question, methods, ways encourage participation by both College faculty and clerkship faculty. Overall, felt to be a worthy, fascinating research effort which should bring much needed attention to the area of teacher training and quality of feedback in medicine.

We then moved on to a mini-journal club discussion (Baernstein A, Liss HK, Carney PA, Elmore JG. Trends in Study Methods Used in Undergraduate Medical Education Research, 1969-2007. JAMA 2007;298(9):1038-1045). We primarily focused our discussion on the mix of study designs (high percentage of studies without a comparison group; impressed by how many researchers were able to conduct randomized control trials in medical education settings).

We discussed the challenges to conducting medical education research (including funding, difficulty securing a 'control' group, focus on interventions but not on collecting pre-data, and opening up clear pathways of communication for researchers to utilize the expertise of faculty members in the Dept of Medical Education when considering an intervention, or research on it).

We had a great turnout (35 people!) for our first forum last Monday.

After group intros, we reviewed the structure of the forum, established some ground-rules, and conducted a 'needs assessment' of the group (click here).

Sara Kim from the Institute for Surgical and Interventional Simulation (ISIS) - http://www.isis.washington.edu/ - gave a dynamic presentation of medical education & research activities ongoing for their team, as well as faculty opportunities and research support. Q&A and discussion followed. Thanks, Sara!

We are delighted at the positive response to a new monthly UW Medical Education Research and Scholarship Discussion Forum.

The first forum will be Monday, February 4 in the Foege Building, Room S110 on the first floor, 12:00-1:15. We will serve a casual lunch and beverages. Sara Kim, faculty member in the Dept of Medical Education and Biomedical Informatics, will describe research activities going on in the Institute for Surgical and Interventional Simulation (ISIS).
